A Chronic Special Needs Plan (or C-SNP) is a type of Medicare Advantage plan designed specifically for individuals who have certain chronic or disabling health conditions. Who Is Eligible?
To enroll in a C-SNP, you must:
- Be enrolled in Medicare Part A and Part B, and
