This is a special type of Medicare Advantage plan designed for someone who has a qualifying chronic condition – including diabetes and certain cardiovascular disorders. Typically, having a chronic health condition can mean you’re disqualified from coverage or have to pay a higher premium in order to get coverage. A Chronic-SNP is opposite. You must have a qualifying condition in order to be eligible to enroll. It’s designed to provide you more cost savings associated with your illness by providing reduced monthly premiums and/or copays related to your chronic condition and giving your extra benefits to help mitigate issues associated with it.
A Chronic-SNP works just like a regular Medicare Advantage plan. It becomes your primary insurance by providing your Medicare Part A and Part B benefits, often includes your Part D prescription drug coverage and gives you extra benefits that Original Medicare doesn’t typically cover.
